During the admission process we strive to gather as much information about our new resident as possible. We start by meeting with our potential resident, family members or caregivers and getting to know them through our evaluation process. We then request information from their Primary Care Physician. With that information we put together an initial memory care plan that is customized for that person’s specific needs. The care plan is negotiable and can be changed at any time as needs and concerns change. An initial Care Conference is held after the resident has lived here for 30 days. At this time the staff Wellness Director, House Manager and Executive Director meet with family members and/or caregivers to discuss any concerns or potential changes in the Care Plan. Care Conferences will then be scheduled every 90 days, or as needed. Our goal is to keep the Care Plan current and comprehensive for all care staff to follow and we value input from all who are involved with the resident.
